Insurance Billing and Payment Policies
To provide excellent service it is necessary that we have accurate information
about you, your employer and your insurance. Please see
Patient Registration Forms for the forms that you will need to complete
prior to your appointment. These forms include a Payment
Policy that you will need to sign. You will also need to bring in a current
copy of your insurance card. Make sure you notify us immediately of any changes
in your insurance.
Our office fees vary depending on the services provided. Please call 222-3225 if
you have any questions about the costs of services or billing information.
Co-payments and Deductibles
Co-payments and deductibles are due at the time of service. Insurance companies
mandate that we strictly collect co-payments at each visit. Our office does not
waive co-payments or deductibles as that may be considered fraudulent by
PPO, Indemnity Insurance and HMO Plans
Our office is a participating provider of many insurance plans. Please call
222-3225 to make sure we are a provider for your insurance plan. Our office is a
provider for Medicare and most secondary insurances.
Our office is not a provider for Medi-Cal, including Medi-Cal
as a secondary insurance.
Our office is not a provider for any Workers Compensation
Third Party & Liens
Our office does not accept Third Party or Lien claims. We will
give you a copy of your bill to submit to the Third Party, but you will need to
pay your bill at the time of service.
If you do not have health insurance
If you do not have health insurance, full payment is required at the time of
service. When full payment is made at the time of service, we do provide a cash
discount. Please call 222-3225 if you have any questions.
