Office Hours

For your convenience, our office is open from 8:00 am to 12:30 pm Monday through Friday, and 2:00 pm to 5:00 pm Monday through Thursday. We are closed on major holidays.

After Hours and Emergencies

In the event of an emergency dial 911. We do have a physician available on-call for after hours. Please dial 222-3225 to reach our answering service.

Appointments

To ensure that services are provided in a timely manner we request that all appointments be scheduled. We attempt to keep appointment slots available in the schedule, so most urgent conditions can be seen the same day. We are unable to see additional family members during your appointment time as we want to be fair to our scheduled patients. So make sure that all family members have appointments if needed. Make sure you are specific with the issues you need addressed, so you have an appropriate length of time scheduled for your appointment. To reschedule or cancel your appointment please call our office at least 24 hours in advance if at all possible, so we may give your appointment time to another patient. Our office reserves the right to terminate services to patients who repeatedly fail to keep appointments without notification.

New Patients

If you are a new patient you will need to complete several patient registration forms. Please make sure you bring your insurance card as we will need to scan your card into your chart. If you do not have insurance information available you will need to pay your bill at the time of service. Please make sure that you come 15-20 minutes before your appointment time to complete paperwork. You may download Patient Registration Forms from our website to complete ahead of time.

Establishing Patients

We do our best to keep to a timely schedule, so please call if you will be more than 10 minutes late. We may need to reschedule your appointment. Please let the receptionist know of any insurance or address/phone changes (including cell phones). It is very important we can bill correctly (or you will get the bill) and can contact you with urgent medical information.

Privacy

Our office works diligently to protect your patient information, per HIPPA regulations. Please see our Patient Privacy Policy for more information. Make sure you notify our office if you want to make changes in your consent for release of medical information.

Prescription Refills

Please choose one local pharmacy for all of your prescriptions. We fax prescriptions from your electronic medical record, which can only identify one pharmacy. Please call your local pharmacy to refill your medication. The pharmacy will fax the prescription request to our office and this process can take up to 48 business hours to complete. To ensure you have enough medication for the week-end make sure you monitor carefully and notify the pharmacy several days before you are due to refill your medication. We are unable to process same day refill requests on Friday.

The most efficient way to refill mail away prescriptions is to call the mail away pharmacy and ask them to fax the prescription refill to our office. We will fax back the request within 48 hours, which is much more efficient then mailing the prescription. We can only fax back forms that the mail away pharmacies originally send us. We can not fax our own prescription forms directly to your mail away pharmacy as our forms do not have all the patient information needed to process your request. If you want to pick up mail away prescriptions, please call our office and leave a message in the appropriate mail box and allow us 48 hours to have your prescriptions available to pick up. If you’d like us to mail you the prescription forms, please send us a request with a self-addressed, stamped envelope.

Many insurance companies have formularies, which are lists of preferred medications. As there are numerous formularies, our office is unable to know what your particular insurance plan covers. Some medications may require prior authorizations or will require you to try other medications first. Prior authorizations are very time consuming, and are frequently denied by insurance companies. Our office only processes prior authorizations on Friday afternoons. Make sure you review your insurance formulary so you know what will be covered. If you are unsure, please feel free to bring your formulary with you to your appointment.

Prescriptions for controlled substances are regulated by governmental agencies and are strictly regulated. These prescriptions must be filled strictly as directed on the prescription. If you are taking controlled substances make sure you only take the medication as directed and do not run out of medication early. Our office policy does not allow us to refill or replace controlled substances that are lost, stolen or requested earlier than indicated. Our office reserves the right to terminate services to patients who obtain controlled substances from multiple providers or multiple pharmacies.

Lab Results

Our office will notify you of all test results ordered by our providers. If you do not receive notification within 1 week of having your tests done, please call 222-3225 to ask about your results.
